Community Health

Community Health

The programs under Community Health cover all community nursing programs across the life cycle. These programs are in place to assist people in making healthy choices and develop healthy and supportive environments.

Community Health Nursing

  • Public Health. Public Health Nurse services include: a prenatal program; a postnatal program; B.U.R.P.S. (a parenting and breastfeeding support program); a Health Checks Program which promotes the health, growth, and development of infants and preschool children; a school health program; and a Community Health Education, Promotion and Screening Program.

  • Home and Community Care. Home and Community Care includes various programs such as: home care nursing; assessments for the institutional placement program; a home support program; palliative care; and a special assistance program which provides health care supplies, various equipment and other health related services to financially eligible persons who have chronic health conditions. This program is available to people of all ages.

  • Regional Diabetes Education.  Regional Diabetes Nurse Educator services include the care of newly diagnosed type 1 and type 2 clients with diabetes, in both pediatric and adult populations, as well as clients with gestational diabetes. Diabetes education also includes insulin initiation, insulin pump therapy and foot care services as needed.  Collaborative clinics are available using a multidiscipline approach. Community education on diabetes and prevention is offered periodically and upon request.
